Well-Being in Residency: A Systematic Review. /clinicianwellbeing/resources/well-being-in-residency-a-systematic-review/ Fri, 09 Aug 2019 21:01:07 +0000 /clinicianwellbeing/?post_type=resources&p=2182 Published in Journal of Graduate Medical Education, the following systematic review identifies factors associated with well-being, summarizes well-being promotion interventions, and provides a framework for future research efforts. Sleep, time away from work, and factors identified in psychological well-being research — autonomy, building of competence, and strong social relatedness — are associated with resident well-being. Moreover, perseverance is found to be predictive of well-being, and greater well-being is associated with increased empathy. Interventions focused on health and coping skills appear to improve well-being. Based on these findings, the author suggests future research should focus on factors identified in cross-sectional studies, including sleep, coping mechanisms, resident autonomy, building competence, and enhanced social relatedness.

The primary outcome was ICU length of stay and physician burnout and secondary outcomes were hospital LOS and mortality rates. The results find that interrupted scheduling with weekend respite resulted in non-significant increases in LOS and mortality. Intensivists experienced significantly higher burnout, work–home life imbalance, and job distress working under the continuous schedule compared with intensivists working under interrupted scheduling (with weekend respite).

The pilot study on oncologists illustrate high satisfaction rates, improvements in confidence in communicating bad news, and decreases in emotional exhaustion scores. This pilot study suggests the CST program seems feasible and potentially effective on improving oncologists’ communication behaviors.

Themes in family medicine help improve understanding of happiness, meaning in life, character strengths, and relevant strategies to develop these ideas. The authors posit that it behooves family physicians to become familiar with and incorporate positive psychology into their practices for better health. Moreover, the article reviews examples of the work in positive psychology, including gratitude, capitalization, “satisfying,” character strengths, and learned optimism and describes potential applications of each area in medical education, physician well-being, and patient care.
